In mandibular gingiva tumor , why level 4 and 6 are not included in the lymph node dissections ?

Too far away: The first group of lymph nodes close to the mandible is what is removed because they are the first set to receive any cancer cells that may float away. If levels 2,3 and 4 lymph nodes are normal(free of tumors) then the more distant ones(level 5 & 6) are not likely to be involved by the cancer. Hence they do not need to be removed in most cases.
