Focused psychological counseling may help you also.

I'm wondering what your family history is for cardiovascular disease, if you are obese, if you exercise or are sedentary, and what caused you to have this fear.  Without knowing more, I can't say how grounded I think your fear is.

Fear of heart attack: My first response is to ask if there is a specific reason you are worried about having a heart attack.   Did someone in your family have an early heart attack?      It is true that more men and women die of a heart attack than anything else.      It is important to know and manage your risk factors.    Avoiding diabetes and metabolic syndrome is critical.  Also, have lab tests for inflammation.

Heart: I am not sure why you would think this would be a heart issue. The right upper back would be a very peculiar referral pattern for cardiac disease. Moreover it would be quite unusual for someone your age to have heart disease but if you have trouble with exercise or ever pass out with activity your heart should be checked promptly.  Right upper back pain more likely related to your lungs or muscles

Relax: Heart palpations can result from and to anxiety.  After appropriate work up (like echocardiogram)..Rest in the comfort that fear is unnecessary. Avoid caffeine, high content sugar drinks and stress situations.  Body awareness and meditation can help. Consider relaxation therapy.

Depends: The presence of the defibrillator itself is not the issue. Persons with a defibrillator have some type of heart disease, most commonly a weak heart muscle. The type of and extent of that disease would determine the advisability of exercise including sexual intercourse. So your physician needs to advise you. Would not want defibrillator to fire during sex, you know. That could be quite a shock!
