Symptoms include: Rash, conjunctivitis, fever/chills and arthralgias. You would certainly be suspicious since it is an autosomal dominant genetic condition. One parent would be affected and sometimes, several siblings.

Yes: It is a genetic disease inherited in an autosomal dominate pattern.  So if you inherit the gene from a parent who has it, you will also have the same problems with hives, fever, and joint pain on cold exposure.

Treat as needed: Familial cold urticaria cannot be cured but it can be treated with allergy meds and minimizing environmental conditions that trigger it like cold etc.

Depends on cause: A/u due to bee, hornet, wasp, fire ant sting can be cured with allergy shots. The patient gets the venom causing a/u in a series of injections. When the allergen can be identified - drug, food, pet, plant - complete avoidance "cures" a/u but accidental exposure results in more symptoms. In 50% of anaphylaxis & 90% of chronic urticaria the cause if never identified so cure is not yet possible.

Urticaria: Numerous controlled studies as well as case reports have demonstrated that Omalizumab can be employed successfully in approximately 75 % of patients with chronic spontaneous urticaria, leading to a dramatic decrement in symptoms with very few side effects. No other drug currently available is comparable, and the success rate in patients resistant to antihistamines is no different.

Xolair now approved: Xolair (omalizumab) is now approved for chronic hives.  It has been used for many years for allergic asthma.  While it is expensive, there are programs to help cover the cost.  This monthyly injection can remarkably control hives.  See a board certified allergist and ask about Xolair for hives.  This is the first FDA approved treatment since antihistamines.

Avyl unsure: The best treatment for hives (urticaria) is antihistamines.  Unsure of "avyl" but most antihistamines work within an hour or two.  There are other medications for hives, but most allergist would suggest that steroids like Prednisone be avoided unless hives are very severe.

Cannot prescribe: We cannot prescribe specific medications here. Generally, for urticaria, Benadryl orally is used, or if it is a more chronic condition, one of the over-the-counter longer acting antihistamines are recommended, such as loratadine or fexofenadine. However, for more specific treatments, please consult your own physician who can see the rash and recommend a specific treatment.

ive had chronic hives for the past 8 months on both lower legs & my right armpit. i think theres a chance it could be cholinergic urticaria. could this be caused by any of these medications - sertraline, co-codamol, tramadol, amphetamine?

: Medications are potential causes of chronic urticarial rash, but there are several possible conditions also associated. Testing for specific allergies, autoimmune disease, cancer and thyroid disease  and CBC, diff  is recommended. Allergic responses to the medications listed should also be considered. Watch for breathing difficulty, weight loss and fatigue.
