Shape of eye: These are terms used by the od or md to describe measurements of the shape of the eye.  They determine your prescription.  They are probably meaningless to you.  Important for the dispenser of your eye wear.

No: These numbers do not translate directly into a snellen acuity number (like 20/20).  The degree of visual clarity is different than the eyeglass strength to achieve that clarity.

Yes: The best number to use is sphere (sph).  If you are nearsighted or myopic the number has a minus sign in front of it.  The higher the number, the worse your uncorrected vision is.
If you are farsighted, the number is a plus sign and the same applies.  The cyl stands for cylinder and is a measure of astigmatism. The higher the number, the worse the astigmatism

Minor distance power: Your distance correction is small and you could function well without correcting that.  You did not state your age, but apparently are over 40 and need near magnification for presbyopia which is age related.  You could get this over the counter.  A bifocal could be made which would give you the convenience of having one eyeglass for all distances.

Read on 180 axis: Think of astigmatism as a foot ball type oval.  The orientation of the points of the football determines the axis which can be horizontal (180 degrees) or vertical at 90 degrees or anything in between.  It is read clockwise with no overlap so there is no set of numbers higher than 180 degrees.

Annual exam: Because your eye is more nearsighted than average, you may have a longer eyeball which can put you at higher lifetime risk for retinal detachment. Have an annual dilated fundus exam to detect any weak spots in the retina. Sometimes the weak spots are repaired prophylactically. If you have warning signs of a dark curtain coming over the vision, flashes of light, or new floaters, go see the eye MD.

Eye glass measures: The sph is the sphere.  This is the amount of nearsightedness or farsightedness, measured in units called diopter.
Cyl is cylinder.  This is the amount of astigmatism. Astigmatism is created when the cornea is not shaped evenly...Egg instead of a ping pong ball. There are other causes of astigmatism.
The axis is an angle that indicates where the astigmatism is located.

No.: The axis for each eye measures which direction the astigmatism (distortion of the cornea and/or the lens) is oriented in, and that doesn't much matter one way or the other.  Even if the axes are markedly different, corrective lenses will compensate for it.  Cylinder measures the degree of the astigmatism (yours is moderate) and that is of greater influence.

None or glasses: You have a mild correction.  Wear glasses if they help.  Talk to your eye doctor.

: You are mildly near sighted and have small astigmatism.  Nothing to worry about.  This year is a small change in prescription. I would renew your eyeglass prescription and get an eye checkup every year
