Big toe and ehl: I'm sorry , but your question really makes no sense .  The ehl tendon runs on top of the first metatarsal for all humans and inserts just before the growth center of the nail on the hallux .

Stretch: If the flexor tendon is too tight it will also cause cramping of the arch area.  You need to stretch out your great toe by hyperextending it gradually (holding your big toe toward your face)  also use some icy hot, etc.  Make sure that you stretch this at least 2 times daily, and before and after exercise.Good support is also needed in the shoes.

Tendon tear: Your MRI indicates a partial tear of a tendon in your arm that is needed for use of your thumb. Whether this can be managed via splinting and allowing it to heal, or if it would benefit from an operation is something a hand surgeon could advise you -- after a more thorough history and physical examination.

Experience counts.: It goes without saying that every patient's condition should be evaluated before applying general rules, but, if the laceration is distal (toward the toes)  to the knot of Henry, then surgical repair is probably best. Find an experienced surgeon.

Overuse and chronic?: If its an overuse injury--running too much perhaps--then rest is of primary importance. This is normally the case so I'll assume this is what's happening with you.
The more you can NOT flex down your big toe the better. That means no flip-flops and wearing stiff soled shoes as much as possible. If it becomes more and more bothersome so its difficult to walk normally see your doctor

The tendon is : Inflamed. It could be you have a small tear or irritation in the tendon. And the surrounding tissue called synovium of the tendon is swollen.

Strength and ROM: Typically after an Achilles repair rehab involves strengthening the muscles of the lower leg/ankle, while increase rom (range of motion) and flexibility.  It is an area that must be rehabbed carefuly, as reinjuring the tendon is possible and certainly not ideal.
