i have daily palpitations for the last 10 years. ecg, echo, holter are all normal.  i sometimes feel my pulse in neck on left side and its bounding accompanied with left chest tightness more after meals. i also have excessive yawning. what to do?

Palpitations: Did you experience palpitations during the EKG or while wearing the Holter monitor?  If so, no cardiac arrhythmia was present and perhaps you're only experiencing a forceful heart beat caused anxiety,caffeine, spices, alcohol,hyperthyroidism. If you didn't feel palpitations while on the monitor, wear an event or loop monitor for an extended period or until you feel palpitations and note them

Probable muscles: There are probably muscle spasms.  The question is why do the muscles cramp?  This can be related to your breathing, head posture, jaw joint or chewing efficiency, airway space, sleep disturbances, etc.  I would check with you physician or dentist to see if the answer to why the muscles cramp is discovered.  Then you will know what and how to treat.

Throat Clearing: Unless the thyroid is asymmetric or huge, would not cause throat clearing. Chronic fatigue and throat clearing, rule out:
acid reflux with reduced sleep
post nasal drip due to chronic sinusitis

generalized aching: consider gum disease or sinus disease.

Goiter?: It depends on the reason for the enlarged thyroid goiter being the most common. I suggest you start by seeing a doctor getting an ultrasound and blood work. Based on those there are several treatment options.

Because thyroid: hormone has a very long half-life, it's levels generally don't "fluctuate". No, they won't cause those symptoms.

Head &amp; Neck Surgeon : You have a goiter that is pressing on your pharynx and even compressing your trachea (your airway).  You need to have it removed surgically ASAP!  Go see MD (not a nurse practitioner) and don't let them dilly dally around.  Get an Ultrasound or a CAT scan of your neck done to verify the source of the mass, and get a referral to a Head and Neck Surgeon ASAP!!!

Depends on location: The thyroid gland resides at the base of the neck on both sides of the trachea (cartilage-encased breathing tube). The neck lymph nodes that can swell up are typically found under the angle of the jaw, directly under the chin, and along the mid neck (along the carotid arteries). A physical exam or ultrasound exam by your doctor can help you distinguish these possibilities.
