PTH vs TSH: Elevated PTH has to do with calcium metabolism and has nothing to do with thyroid or thyroid hormone.

Ane elevated free thyroxine is significant should be associated with a suppressed or low TSH. This would suggest hyperthyroidism is occurring.

PTH can be elevated for a number of reasons including a condition with the parathyroid glands.

Not necessarily: You indicate that the us showed swollen lymph nodes but you don't say how large they were or whether they had normal appearing anatomy. The red and blue color inmdicates blood flow in the node on a doppler picture done to evaluate blood flow. Again amount of flow may be normal or not. Ask your doctor.

Need other lab: Serum calcium levels vary in hyperparathyroidism. Would repeat the calcium test and consider ionized calcium. Consider phosphorus and magnesium also.

Kidney Failure: Kidney failure is another cause of secondary hyperparathyroidism.  When kidneys are not working well, phosphorus levels go up, and calcium levels go down.  These lower calcium level stimulate the parathyroids to make more pth.  Again, even if the calcium levels are in the normal range, they may be low for that person, and cause the PTH to go up.

That's 1 possibility: Besides a parathyroid tumor, it could also be due to low vitamin d levels, or kidney failure.

Correct D deficiency: Vitamin d deficiency reduces intestinal calcium absorption and stimulates parathyroid hormone secretion which maintains the blood calcium in the normal range until it is so severe that a low blood calcium develops.  Correction of d deficiency restores the parathyroid hormone and calcium to normal.

i have an elevated pth with  normal calcium. i also have multiple lung nodules. i now have swelling in my lymph area near the collarbone area?

Get evaluated: You have many findings that warrant further workup.  Elevated PTH may be due to a functioning parathroid adenoma.  An ENT can do that workup for you.  The lung nodules are worrisome and require further workup as well.  May be a completely separate medical issue.  You may need a ct guided biopsy of one of the lung nodules to sort this out.  Seek professional help.

Impossible to say: There are over 100 types if arthritis with RA just one of them. Your rheumatoid factor is really negative so RA is possible but less likely. See a rheumatologist.

Hyper parathyroid : Sounds like you have hyper parathyroidism and you need your parathyroid at least 3 out of 4 of then removed and you should be fine.
