See below: Calcium levels may be on the high normal side on some who have hyperactive parathyroid.  Symptoms are broad, from fatigue to bone pains. Can be primary hyperparathyroidism which is directly from the parathyroid as cause like a nodule or secondary like in kidney disease. Hope this helps.

blood tests have come back with slightly elevated total calcium levels: 10.3-11 mg/dl. over 3 months. pth was tested but without calcium with a value of 36pg/ml. ionized calcium was 5.1. could this be parathyroidism? ive had joint pain for months to?

: According to LabCorp reference range (all laboratories use different ones), normal PTH level is 15-65 pg/mL. Corrected calcium needs to be calculated using a formula accounting for either protein or albumin because calcium is a positively charged anion that binds to them. There are other causes of hypercalcemia that the ordering provide needs to take into account.

Nothing: Primary hyperparathyroidism is diagnosed by demonstrating simultaneous high serum calcium and high pth.  In some patients there is a low vitamin d because PTH increases the metabolism of vitamin d.

Early on: One explanation is that you might have a new onset parathyroid adenoma. It means that even though you have an adenoma it was caught before it caused significant increase in blood calcium levels.

A low parathyroid level will lower calcium.

Kidney Failure: Kidney failure is another cause of secondary hyperparathyroidism.  When kidneys are not working well, phosphorus levels go up, and calcium levels go down.  These lower calcium level stimulate the parathyroids to make more pth.  Again, even if the calcium levels are in the normal range, they may be low for that person, and cause the PTH to go up.

Parathyroid: Hard to answer your question.  Is your doctor wondering about hyperparathyroidism? If so, the ca is high (10.1 is borderline), the PTH is high, and a parathyroid nodule/tumor may be seen on u/s or sestamibi scan.  But sometimes this is not clear.  Your doc (endocrinologist) may want to check your ionized ca, vit d level, your 24 hr urine ca excretion, go over your meds, look for other causes.

Hyperparathyroidism: The parathyroids make a hormone, PTH, which mobilizes calcium from your bones and keeps blood calcium normal.  Overactive parathyroids make too much PTH, raising blood Ca, and leeching Ca out of your bones (Osteoporosis) and too much gets into kidney (kidney stones).  Kidney disease can cause this, but often it is from an abnormal parathyroid gland that must be removed surgically.
