Not necessarily: Some medication are dosed by weight but not others. look at the bottle and follow the instructions given

Standard: There is no benefit for taking more than 600 mg 3 times a day or 500 mg 2 times a day even for a person of your size.  If the NSAIDs are not helping consider adding acetaminophen or seeing your doctor about another agent. Taking NSAIDs at high does does come with risks so it is best to minimize your intake or use for short periods as needed.

Zithromax dosage: The appropriate dose of Azithromycin for a child of 25 Kg is not the same as the dose for the adult.  Higher doses are sometimes used for diseases like Pertussis.

: No, not based on weight for adults…the dose would be the same for age 13 and up. If you have an underlying medical condition, please discuss the dosage and usage with your doctor.

Current opiod dose: We take the dosage of the oral opioid you are already on and convert it to the appropriate equivalent dosage of the Fentanyl patch. It is based upon your tolerance to opioid therapy already. It is not uncommon to have to increase the Fentanyl dose but because it's such a powerful drug that is very harmful in doses too big for your system, we tend to start a little lower and go from there.

Which preparation?: There are several, and whether tablets,  capsules or oral solution?  Tabs or caps 2/dose for adults, not to exceed 8/day. For oral solution 15 ml /dose every 6 hours for children below 12 years, not to exceed 60 ml per day. For adults and children above 12 years, 30 ml every 6 hours not to exceed 120 / day. At any rate you need to read the labels, for dosing, indications and contraindications
