Nerve pain: What you are describing is classic nerve pain. If you have not had back surgery the only other source of scar tissue would be from inflammation like arachnoiditis . Often the burning sensation is due to the release of inflammatory mediators as your trigger points are released thru massage. If it continues to be a problem see your physician.

how long will it take for muscle mid back/low back pain to go away with physical therapy? im doing pt but its taking forever and my muscles feel like there is stuff on them that just stays which causes the pain. what could it be? scar tissue?

: What precipitated the pain? Was there traumatic injury? Depending on the underlying cause, improving muscle strength and flexibility can take weeks if not months. However, exercise and PT is an appropriate approach. You may take pain relief like, acetaminophen, no more than 4 g/day and no alcohol.

Muscles heal: A muscle tear will heal with new muscle tissue but the new tissue will not be exactly the same. The details are too much to write here but in summary there is a loss of elasticity.  If the tear is small, the muscle function should not be affected at all.  The surrounding original tissue can carry the workload and the new tissue contributes adequately. With large tears, the function may be altered.

Reducing scar tissue: Scar tissue massage after gently heating in tub or shower, with a creamy lotion, will gradually reduce stiffness.  Massage should be relatively deep and slow, going different directions -do circles, lines, opposing lines, sideways to original direction.  5 minutes 2 to 3 times a day over several months is normal, may require more if deeper thicker scar.  Don't rub until red or irritated

No: What actually happens is that muscle enlarges as a result of maximum stress being applied, even for a short time, in resistance training or work. This may damage the muscle membranes somewhat as evidenced by increased muscle enzymes in circulation. But this is unrelated to scar, which requires vessels be torn. Ankle Fusion: Ankle fusion (arthrodesis) absolutely will atrophy certain muscles in the foot and leg, as there is no further motion in the ankle joint.  This limits the amount of contracture and use of the muscle, which leads to muscle wasting and atrophy.  Also, you will not return the blood to your heart as effectively, as you rely on skeletal muscle to push the blood back to the heart (Venous insufficiency)

Many possibilities: This could be psychological or physical and would require a detailed exam to elicit the cause of the problem.  See a urologist and make sure there are no anatomical issues, and they could then advise you where to go from there.  Good luck.
