Yes: It means Chronic Kidney Disease, those measurements shouldn't be that high. There are several reasons that could be producing this. My recommendation, go to a specialist, a nephrologist to look for the cause and try to prevent more damage is done to your kidneys.

: Urine cultures are not always accurate, and when symptoms are caused by anaerobic bacteria , may be invalidated by exposure to oxygen. Lupus could be contributing to the presence of increased protein and leucocytes as well. Ask the physician investigating possible lupus for guidance.

Q: how to reduce creatinine level from 4.9, having protein in urine of 67 mg. person also have fasting blood sugar of 160 and now slowly coming to normal?

Lowering creatinine: It seems as if this person has diabetes. Apparently, this person has seen a physician. The creatinine (c) is dependent on a person's sex, age, weight and race. To know what the renal function, a 24 hour urine for (c) clearance and protein needs to be done as well as a renal ultrasound. Go to physician and have the glucose controlled. The c may not go down but may not increase with therapy

You can get it from exercising or the stress of mild illness, and many people get it just from standing up. If your physician isn't concerned, don't be either.
