Keep at it: What do you mean no luck? If there is a waiting list, get on it and be seen. Every state has low-fee/free community health centers. Search online. Go to one and get help from a social worker to obtain health insurance. Great if you can connect to a HealthTap doctor meantime who's in your state!

Work note: Yes  you can call the patient line to discuss the issue that will keep you from work. The doctor can then write the note and you would have access to the note through your medical record right away.

May need to see: psychiatrist for this note if it's a bonafide phobia of yours. It would appear more credible, and verifiable. There's very few professions that you'd be exposed to spiders in reality, since they aren't often seen unless you are in the extermination business. So, realistically, it may be harder to prove it's merit if your job has no exposure to spiders. Most places are clear of these insects.

Dr. exam, then note: A doctor can write a note after doing an in-person exam. Generally, doctors do not write notes such as work-excuse or school-excuse notes, or back-to-work or back-to-school notes, without an exam. That is because a doctor's note is his written, signed, professional opinion, on whether a patient can or cannot do a particular activity. An in-person exam gives confidence to the doctor's opinion.

Doctors note: If the visit was through the paid HealthTap consult service you may want to call or email technical support so that you get instructions to download the visit transcript that will include also possible prescribed meds and if and where the prescription was sent to. Otherwise call the office of the doc who you spoke to earlier.

GP: Your doc would be in best position to assess and decide if a note would be appropriate in your case. Always remember that symptoms have to be reported as early as aware, otherwise notes cannot be issued retroactively based on hearsay. Objective evidence is also needed, such as temperature, rash, etc in order to substantiate the note and for legal purposes.
