Yes : It can be done with just local but i would not recommend it.  I would also try to find a non-operative treatment before attempting surgery.  Do everything you can to avoid surgery.  The recovery is very painful and there are many nonsurgical alternatives that are very effective.

Depends: There are several types of hemorrhoid operations that are tailored depending on the severity of your hemorrhoids and whether they are internal, external or both.  Internal hemorrhoids can be addressed with less pain and short recovery. Simetimes: Hemmorrhoidectomy is a painful procedure but for patients who have not improved despite maximal medical therapy it may offer definitive treatment. Expect 2 weeks of sugnificang discomfort after surgery.
