Yes and unlikely: An overactive thyroid makes the heart beat faster - this is usually sinus tachycardia, but it can also cause aftrial fibrillation or other supraventricular tachycardias. A young person almost never dies of svt unless there is some underlying heart disease.

It is autoimmune: Graves disease is thought to occur due to a combination of factors. It is not well establlished, but the hypothesis are that some people have some genes which prime for autoimmune diseases, then something in the environment triggers the immune system to recognize the thyroid as foreigner. The immune system makes proteins that bind the TSH receptor, increasing the production of thyroid hormones.

No: Once diagnosed with any thyroid condition, the condition known as "thyroid eye disease" can occur at any time, independent of the thyroid status of the body.  So in your case, the puffy eyelids is due to the thyroid condition, not the over-treating or under-treating of the thyroid in the body.
