Not anaphylaxis: Anaphylaxis refers to acute allergic reaction with loss of blood pressure, passing out, tight airway, sudden difficulty breathing. The cause is usually known. You have something quite different. You need a diagnosis-----scabies or whatever----and your Dr should call a dermatologist for you to be seen on an urgent basis.

Infected scabies : Sorry your question is not clear . Yes scabies  can get infected . New pimples would probably be a different thing , Irrespective of that antibiotics would be needed .

Could be scabies: You are describing either scabies or eczema (allergy). If you are also itching in hands and feet it is very likely that it is scabies. In that case all medications are prescription based so you need to see a dr. If not you may try otc Hydrocortisone cream + oral benadryl (diphenhydramine).

It takes 2-3 weeks: It takes 2-3 weeks for scabies symptoms to subside after successful treatment. Even though the treatment killed the scabies mites, your body is still reacting to the dead mites, eggs, and droppings in your skin. Just wait it out; your symptoms will clear up in the next couple of weeks.

Photo would help: The characteristics of the rash are very important to determine the cause. If you can share a photo here on Health Tap- there may be a dermatologist or other doctor who can identify it for you. I wonder if the rash comes and goes or is there all the time? What parts exactly on your body is it found? What exactly does the rash look like? Can you describe it... many Q's but needed to find diagnosis.

: Malathion lotion is usually the next step if 2 treatments with permethrin haven't helped. Sulfur applications and ivermectin given orally are also  other advanced treatments.  I think it would be best to consult a dermatologist, experienced in treating resistant scabies, for accurate diagnosis and appropriate treatment first. You may have to consult one at a university hospital near you.

Scabies: It might be scabies. You should see a Dr to diagnose the rash and give you a prescription for the appropriate medication. Scabies is treated with a prescription cream that you apply at bedtime and leave on overnight, washing it off in the morning. All clothes, sheets, towels will have to be washed and dried in high heat to kill the parasite. All members of your household should be treated.
