Sometimes: DVTs commonly present as swelling and pain in the calf area. However, they're not always associated with these symptoms which can make them more difficult to detect.

That is a good: Question as sometimes it is clinically difficult for us too the physicians. To be sure one can do a blood test called d-dimer test but the best and with immediate results is an ultrasound duplex test. That will immediately show if clot is there or not. Trauma can cause clots and muscle pain. So, for a sudden calf pain one has to see a doctor asap as a clot may need specific attention.

can you feel a deep vein thrombosis forming before it becomes a clot? sometimes when i sit down, i feel a slight ache in my left calf, not painful just a sensation. can this be deep vein?

: Deep vein thrombosis is a condition where a blood clot forms in one or more of the deep veins in the body, usually in the legs. While it is possible to feel some symptoms before a clot forms, it is not always the case.  Some people may feel pain or swelling in the affected leg, while others may have no symptoms at all. In some cases, a DVT may be discovered during a routine medical exam.

Calf pain: Any injury? Did you do any strenous work or run day before it start? If no trauma, no strenous activity, no swelling or rash then try ice for day or two. If it is constant then get evaluated.

Not cause: But the cause of clotting can affect deep and superficial veins.
Stasis, trauma, cancer, coagulopathy, obesity.

No test is ever: 100%, but the combination of a negative d-dimer test(I assume) and ultrasound means that the chance of thrombosis is extremely remote.

i have constant aching pain in left calf and sometimes thigh. worse at night. doc has ruled out lyme disease and deep vein thrombosis. what now?

More evaluation: If your symptoms worsen with exertion (claudication), you may need an evaluation for peripheral arterial disease (PAD).  While these may be simply muscle cramps, be sure to talk with your doctor about sciatica (pain that originates from lower back via nerves traveling to the leg), tendon rupture, and arterial aneurysm.  Your doctor may also want you to see a Rheumatologist.

Requires treatment: Deep vein thrombosis carries the risk of pulmonary embolism (blood clot to the lung) or chronic post-thrombotic symdrome (swelling, pain) if not treated.  Depending on the size, length and duration of the clot treatment might include anticoagulant drugs or a catheter treatment to remove the clot.  Depending on the location of the DVT a filter might be placed to prevent passage to lungs.

Variable: The initial treatment of the DVT is extremely important using proper anticoagulation, compression hose, ambulation, thrombolysis when indicated.  The resultant post thrombotic status will weigh heavily on amount of pain one experiences, and this may be several weeks, several months, and  even a lifetime.
