From scan: It's likely from the vaginally probe. As long as you are not having pain, it's nothing to worry about. If it worsens, or if you start having pain, you need to talk to your OB.

Anovulatory bleeding: Pregnancy is always on the list of possibilities when a woman has abnormal uterine bleeding or cycles.  You may be having anovulatory cycles (cycle without producing an egg from the ovary).  If a urine pregnancy test is negative, i would recommend evaluation by your gynecologist, and treatment will depend on the cause of the abnormal bleeding and your current short-term pregnancy goals.

Preg bleed: Any bleeding during pregnancy needs to be evaluated promptly by your doctor. About 50% of women experience some degree of bleeding in early pregnancy making it very common but not normal. Most are fine but sometimes bleeding can indicate a tubal pregnancy or miscarriage. Get checked.

Taking a pap smear: Scraping the surface of the cervix to take a pap smear sometimes causes a little spotting that you may see in the next few days as a brown discharge. I would not be concerned unless the brown discharge recurs or persists or develops an odor.
