Anticoagulant: Frag in is derived from heparin, a naturally occurring anticoagulant. It is not usually given chronically since oral anticoagulants are easier and cheaper, but there are exceptions such as pregnancy, in some cancer patients, and where oral agents failed.  Fragmin (dalteparin) is often used temporarily to prevent clotting or initiate treatment of a blood clot.  Long term use can cause osteoporosis.

Fragmin (dalteparin): Fragmin (dalteparin) is given by injection. I doubt you'd want to take it for the rest of your life. It's also very expensive. (1 vial is $730).

See your doctor: Fragmin (dalteparin) treatment for a year implies a significant coagulation problem, and your statement suggests that you have had prior pulmonary embolism.  The d-dimer being normal, means there is no significant clot formation, but since you are having pulmonary symptoms again, you should see your doctor for help.

High bleeding risk: To correct bleeding from fragmin, (dalteparin) would use Protamine to help reverse it, although it is not a complete antidote.

None: Fragmin is supposed to work without checking the inr. Depending on what your doctor is doing sometimes they just keep you on Fragmin or switch you back to coumadin (warfarin). I would touch base with your doctor.

See your doctor: The bruise could be pooled blood from the fracture because gravity causes it to accumulate in your feet. However,as you are on fragmin (dalteparin) any bleeding of concern should be discussed w/your doctor.They can determine  the appropriate evaluation &amp; if necessary remove the cast, image and test in the office and replace appropriately. It's important to keep your doctor apprised of bleeding while on Fragmin (dalteparin)

Higher risk of bleed: both drugs together create a powerful effect on inhibiting blood coagulation via different mechanisms but the risk of spontaneous bleeding becomes higher

Not of high concern: There are studies suggesting low molecular weight heparins cause osteoporosis.  The risk is so low (one figure cites 0.91%) that porphylaxis with osteoporsis meds has not been proposed.

Depends: any allergic reaction can be as simple as a skin reaction to a full blown anaphylactic (problems breathing) reaction.

Frag in v ASA: these are two different kinds of medications and they are, by design, administered by different routes.
