Are you asking why?: Pilonidal cysts come from inward-growing hair  penetrating the very thin skin of this area. The midline is an embryological "seam" where the two halves of you fused during development. The skin is so thin there that the curly hair can burrow through into the underlying fat and create a cyst. Because this is a "germy" area, painful flare ups of infection are common. Antibacterial soap +/- surgery!

See your doctor: This string is probably a deep suture coming through the surface of the skin.  The doctor or nurse will simply cut the suture present above the skin. The small hole left by the suture will heal in secondarily with no scarring.

Several: Intense pulsed light (ipl) is really good for
sun damage but especially the brown spots. Chiemical peels can work too depending on the damage as well as on some acne scars.  Other acne scars may need lasers or certain type of in office surgical intervention.
