Weight loss, exercise, low sodium diet, avoiding tobacco and nsaids together with controlled blood pressure and blood sugar will protect your remaining kidney function. Stage 3 CKD: You are correct in that your estimated GFR of 58 ml/min puts you into stage 3 chronic kidney disease. Remember, your GFR is estimated. The actual result may be greater, or lower, than the egfr. To get your acutal gfr, have a 24 hour urine for creatinine clearance. Ask the physician who orders this test to have proteinuria (p) assess in that sample. If p is present, it may needed to be treated.

Creatinine: Normal creatinine ranges from 0.6 -1.3.
This can vary from lab to lab age and sex.  Dehydration, exercise for 2 days prior to testing and ingestion of &gt; 8oz of meat can erroneously elevate levels.

good evening doctors. should a 63 year-old male with kidney issues (serum creatinine 1.49 now, 1.33 previously) with low vitamin d (26 now, 22 previously) take vitamin d3? one of my doctors told me yes and one told me that there is no reason to do so?

: Your Vitamin D levels appear borderline low, and most physicians would recommend supplementing vitamin D3.  Typically, would advise seeking a blood level of 50-70 ng/ ml as an optimal range.  This could be done through contact with sunshine and/or use of the D3 capsules.

See nephrologist: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, an estimated glomerular filtration is a better indicator of kidney function. Unless you are a muscular man engaged in heavy physical activity, it would be prudent to see a nephrologist for evaluation and advice.

Control and prevent.: Treatment is aimed at controlling the underlying disease which is causing his dysfunction ( diabetes, hypertension, etc.) and in preventing further damage to the kidneys by avoiding any kidney toxic medications and in potentially taking appropriate medications such as angiotensin converting enzyme inhibitors.  Sounds like you have already talked to someone about this.

Single Kidney: You did not mention why your kidney was removed. Please get RENAL Ultrasound to ascertain there are no anatomic problems, such as hydronephrosis, with the remaining kidney. Also infection or stones (obstruction) can also compromise the residual kidney function. Please see a qualified Nephrologist (kidney Specialist)

Possible: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, it would be prudent to repeat the test after proper hydration,.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Kidney failure : Kidney failure can be caused by a multitude of things including drugs, metals etc . Your father needs to see a kidney specialist to determine the correct regemin of treatment. Good Luck
