i am60 y/o female i had a blood test done on 10/16/12. just got results.  total cpk was 621. should i get this rechecked right away? what should i do?

Muscle injury?: Are you taking any medications like statins? Did you have any injury to muscles? You should consult your doctor who ordered the test as he likely has the information needed for proper interpretation of the results. All lab results need to be interpreted in the clinical context. Need more info: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, 1+ anisocytosis is not a cause for concern. CK elevation can occur from just having exercised before blood was drawn. Why were these tests done? Do you have any symptoms?

If : If the er could not uncover the reason why he went into rhabdomyolysis, then you may never know. I will list some common causes for you, maybe this will help:

muscle trauma or crush injury
burns(severe)
physical torture 
prolonged lying down 
prolonged coma
severe muscle contractions from seizures
cocaine use 
increased body temperature
extreme physical activity 
drug and alcohol intoxication
low electrolytes
genetic muscle diseases 
near drowning 
low body temperature), 
medications: most commonly statins, which are used to treat high cholesterol; psychiatric medications, anesthesia medications, HIV medications, colchicine(used for gout)
viruses and some bacteria
untreated  hypothyroidism (low thyroid level)
myopathies(muscle diorders)
snake venom

Muscle injury: Creatinine kinase is released from injured or dead muscle cells, be those skeletal, cardiac of smooth muscle cells.  Renal failure is also associtated with modestly elevated levels.

Muscular dystrophy: Muscular dystrophy is a genetic disorder affecting the muscles.  There are different types with different modes of inheritance, age of onset and severity. The CK (or CPK), creatinine kinase are usually elevated in any one. There are different isoenzymes for CK yet it is the total CK that is measured and important in muscular dystrophy. The final diagnosis is made by genetic testing or  other tests

Context: If you are a sedentary adult with coronary risk factors and chest pain, you are probably having a myocardial infarction / heart attack. If you are a athletic adult who has just completed a rough strength workout in the gym or a homeowner who has spent the weekend chopping and carrying lumber, you are fine. In my population of physically-active med students, a majority have had elevated ck's.

Healthy: Despite the laboratory's reported parameters, there is no low end for ck - it is perfectly healthy to have a value that is less than the "lower limits of normal" for this test and relates to your size, gender, age, and muscle mass. There aren't any diseases that cause this.

should i be worried with a 365 cpk , normally it is 120, i started to left weights between blood test, 70 yrs old on  atorvastatin?

High CPK: With your increased CPK on statins, i would stop them and have you see your physician. Statins can cause muscle destruction leading to an elevated cpk. Tell your doctor if you have muscle pains or weakness. You and your physician may decide that you need an alternative therapy for your high cholesterol levels and may need to stop the statin.

i have rhabdomyolysis. with every blood test, my kidneys are okay. as my cpk went down from 126,000 to 721, should i worry of my kidneys still???

If your kidneys are: ok at this point it is unlikely they will have a problem as your CPK reduces. Discuss your concerns with your health care provider who knows all the specifics of your case and renal function.

Find cause: You are going to be fine, but be sure you know why you had the rhabdomyolysis in the first place. I did this to myself by running 14 miles after having laid off all winter. I trust you are not doing cocaine. If you were taking a statin, re-evaluate. There are many other possibilities. Best wishes.
