Follow doctor's: advise. INR of 4.6 is higher than desired. Do not change your diet without checking with your doctor as you could easily go in the opposite direction and not be sufficiently anti-coagulated.

Bleeding: Excessive bleeding is the danger of too much coumadin (warfarin).  Bleeding into the skin can cause bruising and pain.  Into muscle can cause a hematoma and stiffness and could lead to a compartment syndrome.  Into a joint, can cause swelling, stiffness, and pain.  In the stomach and intestines, can anemia and gastrointestinal bleeding black, tarry stools and diarrhea.  Into brain can cause a stroke.

Bleeding. : This medication can cause serious bleeding if it affects your blood clotting proteins too much (shown by unusually high inr lab results). Even if your doctor stops your medication, this risk of bleeding can continue for up to a week. Tell your doctor right away if you have any signs of serious bleeding, including: unusual pain/swelling/discomfort/easy bruising, prolonged bleeding from cuts.

AllegraD / warfarin : I can find no warnings in the epocrates database for either fexfenadine or pseudoephedrine, the two ingredients in Allegra (fexofenadine) d regarding interactions with warfarin (coumadin) .  But just to be sure, check with your pharmacist and your cardiologist. If you are taking Coumadin for a heart condition, your doctor might prefer you take plain allegra (fexofenadine). The d part, sudafed, is a cardiac stimulant.

Reason?: It depends on why you are on it.  If you were on it for a blood clot that resolved, it can simply be stopped when your doctor says it is ok to do so.  If it is being interrupted to allow an invasive procedure off blood thinners, in some cases "bridging" with a shorter acting blood thinner may be necessary.  Check with your doctor.
