Moderate: Dosing is idiosyncratic, that is, depends upon one's hepatic metabolism which is very difficult to know with certainty, so most mds start low and titrate to effect.

Not Really: Some patient require higher dose as hepatic metabolism is variable. Also, intake of vitamin k containing food will impact Coumadin (warfarin) dose.
