Reason?: It depends on why you are on it. If you were on it for a blood clot that resolved, it can simply be stopped when your doctor says it is ok to do so. If it is being interrupted to allow an invasive procedure off blood thinners, in some cases "bridging" with a shorter acting blood thinner may be necessary. Check with your doctor.
