You need help: Do not find specific meds for diabetes or know what is occurring with your blood glucose measures.  Strongly urge you see your doctor, as optimal control of blood sugar, use of nerve pain med such as Lyrica, (pregabalin) and adding medical food called MetanX will help immensely.

Sounds like you have: Arthritis, or a meniscal tear. The fall might be responsible for the arthritis, but i think its the daily activities with subtle injuries in a susceptible person resulting in a meniscal tear. Its not a nerve issue.
See an orthopod and I am sure he'll help you.
In the meantime, use a cane for support, as I have seen too many people falling ; making it worse.

Could be sciatica: Sciatica can certainly last for 4 weeks, but other things can cause pain in one leg only. You should see your doctor for a full evaluation.

Yes: It could be due to that,  But it also could be due to diabetes, heavy metal poisoning and other nutritional deficiencies.  I think you need to return or see another doctor for a complete workup including labs and possibly an imaging study on your back.

Local neuropathy: A neuropathy that affects only one leg sugests an injury to a nerve that supplies your left leg.  It could be related to your prior surgery.  If there are motor problems with your left leg (for example, a foot drop on that side) you should consult a neurologist to help find the cause and arrange for treatment.
