Most docs get concerned with "resting" heart rates below 50 or above 120.Discuss with your doctor!

Resting Rate 100 : Rapid heart rate is not a common side effect seen when taking Concerta (generic methylphenidate), but it can occur.  60 to 100 beats per minute (BPM) is considered a normal resting adult heart rate. It is very important to establish a patient's baseline heart rate prior to treatment as some patients normally have heart rates greater than 100.

Worrisome: Any kid that age with a heart rate in the 35-40/m range is worrisome. I would want them evaluated by a heart doctor (cardiologist for a conduction defect regardless of what meds they may be taking.

Yes : If you have a normal echocardiogram 2 years ago and was normal and there is no changes in your history and physical exam. Echo will be the same. Concerta is a stimulant of CNS activity and one of the serious complications is arrhythmias but not a common one. It's use for the treatment of adhd.

Not neccessarrily: Energy drinks are high in caffeine and you should not have any problem with concerta, you may start with small dose and gradually increase if needed

Discontinue dextro: Although there are no real drug drug interaction between Concerta and dextromerthorphan, but if  its causing side effects please discontinue the cough medication. At times a simple antihistamine like Benadryl (diphenhydramine) or Loratadine helps with cough. Consult your physician always.

Concerta & heart: Since cardiac arrhythmias r possible serious reaction to methylphenidate & rapid heart beat & palpitations r common reactions, your cardiac function should be monitored closely for changes. I would also b very cautious about using caffeinated products (including NSAID combinations) & decongestants (the "d" in claritin-d, zyrtec-d, etc) as they may further stimulate rapid heart beat.

Unrelated: Anything you took 3 weeks ago is not having any current effect. Don't assume you have pvcs - pacs are far more common in your age group. Are you stressed? Drinking cola, coffee or other caffeine drinks? Using any form of stimulant (herbal, over the counter, natural?), getting plenty of sleep? If still troubled, a 24 hour holter exam will reveal the exact problem and guide therapy. Some palps r nl.
