The treatment is typically chemotherapy, with/without embolization.  The presence of an encephalopathic clinical picture also portends to a guarded at best prognosis.  Please consult with a specialist care team to determine therapeutic options.

6 months: Every 6 months with an ultrasound and AFP blood test.  We alternate ultrasound with annual ct scan or MRI of the liver at our center as the ultrasound has some limitations in cancer detection.

Cirrhosis and Cancer: It sounds like he has a prudent doctor who is caring well for him. The risk of liver cancer (hepatocellular carcinoma) is higher in patients with cirrhosis, but the degree of risk varies by the cause. It is highest in those with hepatitis B or C as the underlying cause but lower in patients with alpha-1-antitrypsin deficiency-related cirrhosis. High in the case of HBV-related cancer means prhaps 60x higher than normal uninfected people. Duration of infection also matters.

Cell : Cell growth and death is always occurring all over our body, including liver cells.  Cirrhosis results in disrupted anatomy of the cells of the liver.  Over time this affects dna.  Alterations in dna can result in mutations of genes that make liver cells.  When mutations occur there is a risk of a mutation involved in cell programming, to include programmed cell death, which is supposed to occur.  When the genes that program the liver cell to die fail to work, the cell can multiply/grow out of control, which is what cancer is.  This growth requires a lot of energy and nutrients, supplied by blood.  In time, this growth overtakes the body's ability to compensate for the increase in nutrient requirements and death ensues.

Usually: Primary liver cancer (hepatocellular carcinoma, or HCC) can usually be seen on modern ct scan, or PET-ct scan. It is usually seen as a separate mass within a nodular, shrunken liver if cirrhosis is present. Definitive diagnosis is usually made with a biopsy of the tissue, often done by an interventional radiologist. No scan however is perfect. Hope this helps.
