Yes: Cirrhosis is a risk factor for developing liver cancer. But it happens in a minority of patients, most die due to other complications of cirrhosis.

Depends: If the cause of the cirrhosis is hepatitis b or hepatitis c or hemochromatosis, the risk for cancer is high -- cirrhosis or no. If it is alcoholism, autoimmune hepatitis, primarily biliary cirrhosis, or metabolic syndrome, the risk is much less. Much of what we used to consider irreversible about cirrhosis is now more hopeful, and perhaps you'll receive a transplant.
