LAB AND XRAY: Stone(s) in the common bile duct (choledocholithiasis) present with pain and/or jaundice, and can cause pancreatitis, a severe inflammation of the pancreas gland.  Blood tests, ultrasound, mrcp (magnetic resonance xray of the bile duct, ) and/or ERCP (endoscopic exam of the bile and pancreatic ducts) are all avenues used to evaluated cbd stones. Common duct stone: The liver makes bile, the gallbladder stores it, and the common bile duct drains it into the intestine (duodenum).  Sometimes a stone from the gallbladder ends up in the common duct, or less commonly, a stone forms there after the gallbladder is removed. Depending on the presence or absence of the gallbladder, treatment may be surgical or primarily by gastroenterologists, or a combination of both.

Gallstones: In western countries, most cases of choledocholithiasis are a result of the passage of gallstones from the gallbladder into the common bile duct. There are many risk factors for gallstone formation, such as being a multiparous female, obese, and over age 40. Other risk factors include oral contraceptive use, decreased physical activity, diabetes mellitus, and family history and genetics.

Pain, Jaundice: Pain from choledocholithiasis (common bile duct stones) will be identical to the pain from gallstones, since both cause pain by blocking the outflow of bile from the gallbladder.  However, since the liver is also blocked, one will be more prone to infections in the liver and jaundice; lastly, these stones may cause pancreatitis, which can cause severe mid-abdominal pain.

Bile duct stones: The choledochous means bile duct in latin.  Lithiasis is presence of stones.  Hence, choledocholitiasis is stones in the bile duct.  These stones require treatment usually an endoscopic procedure called  ERCP is done for stone extraction followed by gallbaldder removal.

Stones in bile ducts: Stones lodged in the bile ducts may eventually pass into the small bowel on their own. However, you need to be proactive if these are causing symptoms, or you have elevated bilirubin &/or dilation of the bile ducts on recent imaging. Obstructing stones (depending on location, size, biliary anatomy) may promote infection, pancreatic &/or liver inflammation & need to be removed (typically by ercp).

Choledocholithiasis: Is a stone or stones in the main duct draining bile from the gallbladder and liver. These may or may not cause blockage of this duct. Mirizzi syndrome is blockage of the same duct by a stone not in the main bile duct but in the smaller (and close by) duct just draining the gallbladder. The blockage is usually from swelling around the stone. See graphic.

Sort of: Choledocholithiasis refers to gall stones in the common bile duct. A few cases are due to stones forming directly in the duct. Most cases occur when stones that formed in the gall bladder pass into the bile duct. There are many risk factors for producing gall stones and positive family history is one. Whether a tendency to pass those stones into the common duct can be inherited is less clear.

: Depends on the solubility of your bile. Rarely studied.  Review your family history of Gall bladder disorders and discuss with local Drs who know more about this in folks like you who live where you live.
