Sarcoidoisis?: There are several causes for enlargement of the lymph nodes in the region in which the trachea (air tube) divides into main stem bronchi (smaller air tubes).  Sometimes this can be caused by a condition known as sarcoidosis, but this may not be what your mom has.  This should be discussed with her doctor at length and if you are not satisfied and haven't seen a pulmonologist, request a consult.

Ct imaging: Adenopathies are suggested by x-rays and eventually confirmed or not by CT scan. All tests should be ordered in context of actual symptoms and with clear goals. If any acute condition  time should be allowed between tests.

Several causes: Lymphadenopathy can be due to both benign and malignant causes. Benign causes include sarcoidosis, tuberculosis, and other disorders. Cancer or lymphoma can cause lymphadenopathy. This needs to be further evaluated by a physical examination, possibly a ct scan, and possibly a biopsy.

can mri detect lymphadenopathy in chest or ct is the only option?

Yes: MRI can easily detect differences in soft tissue structure so that lymph nodes are easily identified. CAT scan can show organ structures easily and the amount of radiation with the newer units is minimal.

i have pneumonia, my chest x-ray is showing a hilar mass or hilar lymphadenopathy. would this be caused my the pneumonia?

Talk to your doctor: This is a potentially very severe situation.  The adenopathy or hilar mass may be causing a post-obstructive pneumonia, or the hilar lesions and the pulmonary infiltrates may be part of a single entity such as sarcoidosis.   This should be discussed at length with the doctor caring for you.

No: It needs to be investigated hilar mass could be enlarged nodes unlikely to be due to pneumonia.

Infected node: Supraclavicular nodes are either primarily involved with infectious process derived from lung but ruled out by chest film.  Occassional neoplastic spread to node can cause necrosis but more frequently enlarges.  Best approach is to biopsy node and culture the necrotic area.  Cause if no malignancy present will be detemined by organism grown out.

mom had ct chest after having bronchitis showed mild mediastinal & left hilar lymphadenopathy,they did biopsy it was neg, no symptoms,why biopsy again?

Biopsy: I do not know when the initial biopsy was done.  However, although the nodes may be negative on the initial biopsy if the hilar nodes remain swollen or are enlarging that may warrant another biopsy.

central necrosis.

Lymphnodes: not necessarily TB could be sarcoid or just a degenerating node  stay alert to changes and see your doctor
