I don't think so: whether prescribed or not, chronic opioid therapy has effects on many things needed for driving, and while regular drivers licenses are not a problem, a commercial license may well be.  It may depend on the state.  Usually there is a booklet available from each state regarding their requirements, and I would start with getting and reviewing that. Talk to your prescribing physician and get a letter

It depends: In general, a follicle that doesn't release before day 18 is considered lower quality. However I have seen people get pregnant late in their cycles, with a late ovulation, and it turned out fine. There's no way to give you a percent chance. If you are doing fertility medications, i would trust your doctor's opinion about whether or not to continue injections if it isn't working as expected.

OPKs are variable: Ovulation predictor kits are variable in their reliability and accuracy. I am a big fan of them, but know that some work better than others. I don't know of a particular brand that seems better than others, but have been told that the electronic ones tend to be more reliable.

Likely nothing: Monoclonal B cell lymphocytosis is a small population of malignant lymphocyte in the blood. A level of 0.4% is tiny, and likely not going to turn into anything bad. However, these cells are the type that make up chronic lymphocytic leukemia (in far higher numbers)and small lymphocytic lymphoma. It would be good to rule out lymphoma, and follow blood counts every 3-6 months. No treatment needed.

Varies Person&State: Like most legal issues, varies with both the law in each state & individual characteristics. Most people taking Insulin are type 2, a bad treatment option because it drives appetite, obesity & intermittent low glucose, sometimes dangerous to them & others. Thus check out the issues; stop all carb intake & if type 2: titrate up victoza, (liraglutide) +/- amylin, +/- metformin: far better tx & check state laws.

Try this: I discourage testing twice a day: there's no reason to, and most OPK are intended to be used in the mornings. Read the directions again for your kit.

Needs more testing: I assume this is in peripheral blood, you need more work up ask to be referred to a Hematologist

Hard to give: A definite diagnosis with a sketchy lab result, clinical correlation and more information are needed, ask the doctor who ordered the test
