Restoration: A cavity on the gum line can be fixed with a filling... Usually a tooth colored filling is used for esthetic reasons, but a silver filling can be used on the posterior teeth, too.

: The best solution is to go to a dentist for treatment. If tartar it can be scraped off, if a cavity it will show up on an x-ray Poor oral hygiene will cause both.

Absolutely: It will trap food and bacteria which can contribute to gum recession, bad breath, and premature failure of the restoration. Have your dentist smooth down the edge in question.

Seek second opinion: Must see a radiograph and do a clinical examination to answer.  Go for a second opinion if you feel uncomfortable with the diagnosis.  It may be more cost effective to extract the tooth and place an implant, if the tooth is deemed non-salvagable.  If it is restorable, crown lengthening procedure, possible root canal and post and possible new crown may be options.

Yes. : Twelve cavities are alot. Most cervical caries at gum line can be fixed. A tooth colored bonded resin filling will look good, may help to close gaps, and can last for years with a better care and improved diet. However some of your problems might be related to gum disease which if not treated may lead to tooth loss. Ask your dentist.

It : It wasn't mentioned if you have symptoms, e.g. Severe pain, why  a root canal was recommended. Assuming the tooth is healthy, if you are my patient, i will use my laser to do gum reduction (gingivectomy) after application of a strong topical anesthetic, provided it won't violate the biologic width (which is within 3mm), to access and restore the small cavity. In case there is already a pulpal involvement, (e.g. A narrow opening but  deep enough to communicate to the pulp), a root canal has to be done. I will recommend implant vs. Bridge to preserve the bone integrity, easier flossing, and capacity to chew hard foods without traumatizing the abutments where the pontic (floating crown) has to be connected.

The success and longevity of this treatment will depend on a number of factors including your occlusion (if the bite is balanced), oral hygiene, habits, and diet.
