Petechiae: Punctate cutaneous red marks, characterize very low platelets...75k is low, i've seen it a lot, and cannot recall ever seeing petechiae at that level (not low enough).  Splinter hemorrhage is a sine of bacterial endocarditis...A lesson from physical diagnosis, but I have also never seen in a 35 year career.

Rheumatic heart dis: Rheumatic diseases themselves are not associated with splinter hemorrhage of the nails, but any rheumatic disease such as rheumatic fever that damages the heart valves can lead to bacterial endocarditis which does. The other major cause is injury.

Splinter hemorrhages: Are not uncommon.  Most common cause is repeated trauma (it can be trivial) to the nail bed. The classic case is when they are seen in endocarditis. I did look up and found a case of p. vera that presented as extensive splinter hemorrhages.  So I guess, yes, it might be the cause in your case.

: First I recommend having a dermatologist examine his fingernails to determine whether these fingernail findings are abnormal ( vertical ridges are not) &amp; attributable to any systematic disease or deficiencies.. Splinter hemorrhages are often caused by trauma but infection on a a heart valve is a possible cause. The dermatologist will order any appropriate tests after the assessment.

Yes: Your observed hemorrhages must be a local injury. If due to a heart infection or systemic disease you would have serious symptoms.

: Splinter hemorrhages can happen from traumatizing the fingernail.  Do you have something that you believe is splinter hemorrhage?  If you bite or pick fingernails, it is important to stop this habit. It is very hard on your fingertips AND teeth.  Especially now that we are still very concerned about Covid-19 infection, avoid placing hands/fingers on any part of the face, including the mouth.

(Basis for concern, as 1 piece of diagnostic pattern, is recurring, multiple fingers &amp; toes without apparent basis for injury plus unexplained, non-localizing symptoms &amp; persistent fever for weeks which may be a result/sign of endocarditis or other intravascular infection shedding infected microclots through the arteries &amp; creating the finger splinter hemorrhages.)
