what is a posterior subcapsular cataract and ischemia of the optic nerve?

2 different things: There are many types of cataract.  Posterior subcapsular cataract refers to cloudiness on the back part of the lens just under the capsule.  This may be caused by injury, steroids, elevated blood sugar and aging.  Ischemia of the optic nerve means that there is or was an episode of poor blood flow to the nerve which may result in vision loss.

Back of lens: This is a change in the lens transparency from the growth of material under the back, central portion of the lens.  It is typical as a complication of systemic diseases and medications like high dose steroids.  It can occur naturally and usually is more common in slightly younger patients than ordinary senile cataracts.

It can be but low: Uveitis can be associated with Thyroid disease but it is ususlly a very low percent around 2%. It is been found mainly in poorly controlled hyper thyroidism. You could be check to see if you have any other autoimmune phenomena that might explain your case. Your physician should be able to help you with this.

Destabilization: The natural lens and its supports stabilize the front of the eye and vitreous.  Removal of the thick natural lens and substitution with a thin artificial lens, increases the chance for the vitreous to move in ways that can destabilize the attachments of the vitreous over the retina.  If there are weak places, this can increase the chance of detachment, even in the best of cataract surgeries.

Very small : As vitreous traction is the main cause for retinal detachment following cataract surgery, if the vitreous is already separated, there would be no traction and thus the chance of a detachment would be very small.

should i be seen?

Yes: it may be nothing significant but with your ocular history, you should have a dilated examination of the eye.

Sometimes: Cataracts interfere with vision making you work harder to see.  This could increase the feeling of fatigue.

how to know when do cataracts cause blindness?

Accepted Answer

When advanced: Cataracts cause a progressive lowering of the vision but do  not cause permanent blindness since they can be removed even when very advanced.  This assumes that everything else in the eye is normal except for the cataract.  Cataracts are the leading cause of blindness on earth since surgery is unavailable  in many poorer countries.
