what are the causes of brain fog and breathlessness upon standing? no racing heart or dizziness, just heaviness. comes and goes, worse when tired.

Forgetting to breath: Without any other information available about your condition, the one thing that comes to my mind is failure to remember to breathe when you are tired. This would lower your oxygen in your blood and cause both brain fog and a feeling of heaviness in your chest along with breathlessness.  This can occur in sleep apnea and i recommend a sleep medicine consult.

can neck tension and anxiety cause dizziness and brain fog?

See your doctor : Your symptoms can be due to a multitude of medical conditions. These need to be ruled out before we assign your symptoms to anxiety or "stress". Please see your doctor for a complete history and physical exam to include comprehensive lab panel. Your symptoms could be due to diabetes, thyroid dysfunction, medications, neurological illness, etc. Best wishes!

Stress related?: It may be stress related if you have appropriately ruled out medical causes by visiting appropriate MD's and following their recommendations. If your primary care physician can find no medical issues/concerns, a referral to a psychologist for stress reduction, relaxation training and overall wellness sessions may be helpful.

my connective tissue disease causes dizziness/lighthead, brain fog, black outs which has been mostly resolved by methotrexate.what is likely cause?

If: If the Methotrexate worked, perhaps it lowered levels of inflammation that were making you feel bad.

i've been taking nexium (esomeprazole) and developed bad dizziness, nauseau, neck pressure and brain fog.  could the nexium (esomeprazole) cause this?

Possible: I would discuss this with your doctor who recommended the nexium (esomeprazole). Alternatively you could just discontinue the drug and see if your symptoms resolve.

this month i have been suffering with symptoms of brain fog, dizziness, nausea, and intense anxiety. i noticed that on my recent blood work my igf-1 is elevated and is at 417 ng/ml. is this level cause for concern and can it cause these symptoms?

: Would need to see normal values for that lab and see and examine you to be able to help much.  Not likely to be causing your symptoms. Would discuss with Dr. who ordered the tests.

Cervical spine spurs: The cervical bulges will not affect the dizziness , lightheadedness and brain fog.  You might get some pain down your arms or referred pain to the neck.  See your local ent make sure that the cause is not a ear problem.  Good luck and feel better!

Otoscope?: An otoscope can't visualize the sinuses.  Go see your doctor to see what additional testing might be indicated.
