Menopause.: Those levels being elevated would usually be the biochemical correlate of the menopause, and would then be normal in this context.

Ischemia. : Monocular ischemia most likely causes shots a in one eye.  Plaquenil (hydroxychloroquine) and pituitary adenoma causes symmetrical bilateral schotoma.

how to treat dysfunction of hpa axis? mri pituitary normal, short synathcen test normal, pelvic ultrasound normal. docs think previous low weight/current inflammatory disease may be cause. estradiol, progesterone low, t4 and testosterone low normal?

: If your pituitary MRI was normal, get a referral to a reproductive endocrinologist for evaluation of estrogen/progesterone levels, as such may just be drawn at a point in your cycle where values are in fact lower (post ovulatory or the first day or two of your menstruation).

Yes: There are congenital pituitary disorders, although rare. You can have situations where the entire pituitary is not working, or just certain hormones, say, just the thyroid stimulating hormone or growth hormone. We usually don't know what actually happens to cause these things. Many things have to happen just perfectly while a baby developing, and steps can go wrong anywhere along the way.

Not likely: It may depend on what "diet pills" you are talking about, but most over the counter pills will not affect your thyroid or pituitary gland.
