It depends..: The flaking/scaling on these areas are actually dead scabs and so should not cause much problem..Unless peeling it off also peels of your skin/causing bleeding etc....Which may expose it to infection etc...So be careful...

Need to be checked: Peeling skin can be a sign of several different things.  It could be a reaction to the antibiotics or from the infection that the antibiotics are treating. 

More concerning is the possibility that your child has a something called kawasaki disease.  If that is the case, he doesn't need antibiotics, he needs to be seen by a pediatric specialist.  I would take him to a pediatrician or pediatric er.

Get it checked: I recommend having it evaluated.  It could be a number of things.  Eczema, psoriasis, allergy, fungus, just to name a few.

Dry skin: You are more likely to get an infection from the cracks in the dry flaky skin than from the moisturized repaired skin. Use a first aid cream for the first 24 hours then use either eucerin cream or a & d ointment thereafter.

Always like that?: Healthy children in the U.S. Usually do not have nail problems related to malnutrition, so soft nails are more likely be be familial. A parent can ask the primary care doctor or pediatrician to look at the nails, and refer to a pediatric dermatologist if needed. The doctor will want to know if the child's nails have always been soft.
