Neuropathy: Yes its quite possible that it will not be picked up on EMG. Peripheral neuropathy is a small fiber disease - EMG's are not very specific with small fiber disease. The best is to do a non-invasive Nerve Conduction Velocity (NCV) study which is a more specific test for small fiber function. - keep in mind that there is a great deal of controversy on the application of EMG's

Depends: This can be a systemic disease process (neuropathy, hiv) affecting multiple nerves at once. Carpal tunnel affect the hand specifically, not the arm. You can have two pinched nerves possibly, but one tries to explain symptoms by the most simple explanation. Consider spinal cord issues, which would affect arm and leg simultaneously.

Need an examination: You need to have a medical examination to help you find the cause of your pains. What is sll? Please do not confuse us with use of abbreviations...They mean different things to different people. So lets keep it simple and keep it clear.

Numbness: Carpal tunnel syndrome may cause numbness, tingling or pain in the thumb, index, middle, and sometimes thumb side of the fourth which which can also be associated with numbness going up to the elbow and occasionally the entire arm. Tendonitis is local pain over the involved tendon which may radiate. Carpal tunnel syndrome is due to a compressed median nerve in the carpal tunnel at the wrist.

CTS = Mononeuropathy: Cts refers to the condition brought on by compression of the median nerve as it travels through the carpal tunnel (compressive focal mono-neuropathy). The term neuropathy is a general term referring to various conditions and multiple etiologies. Cts is a form of a neuropathy, but not the only neuropathy that can affect the median nerve.

Could be either: Diabetic neuropathy usually affects the feet before it affects the hands and arms. However, your symptoms could be coming either from carpal tunnel or from peripheral neuropathy. 

Symptoms from carpal tunnel are usually most intense in the thumb, index and middle fingers and are worsened by repetitive motion. Clumsiness, and weakness in the hands also occurs commonly in cts. An EMG is needed.

how can you the difference between nerve damage, bible cyst, tendonitis, and carpal tunnel?

All are different: "bible cysts" refer to a mass or swelling on the top of the wrist which is a ganglion.  It is usually round and non-tender. Tendinitis in the back of the hand or wrist is more diffuse and at times painful.  Carpal tunnel syndrome occurs with pressure on the median nerve on the palm side of the wrist with numbness, pain, and tingling.
