You develop Sx's: which may include: headache, fever, shivering, sweating, fatigue, nausea, vomiting, dry cough, muscle/joint/back pain &amp; enlarged spleen. There is a cyclic pattern of being very cold &amp; shivering followed by feeling very hot and sweating. Additionally one can develop jaundice, blood in urine, anemia, eye problems (nystagmus, problems with eye movements), seizures &amp; coma.

Yes, still serious: Malaria symptoms show up a week to a few months after being infected by a mosquito bite. Fevers, chills, fatigue, swelling of the spleen, etc... Usually occur as red blood cells break apart. With treatment, the symptoms should go away in a couple of weeks. However, without treatment, and depending on the type of malaria, an infected person can have symptoms for a few years, up to a few decades.

No: I assume you mean in the usa.  There is currently no malaria being transmitted in the usa.  Last year there were some cases transmitted in key west florida.  Intermittantly there are cases transmitted in southern texas.
Of course being "from the United States"
won't protect you if you go into countries where malaria is endemic. The parasite doesn't respect our us passports.

Malaria recurrence: It's possible that one type of malaria could 'come back' years later - that's Plasmodium malariae or P. malariae.  There are cases of (veterans usually) who've gone overseas - got sick with malaria, then recovered, then YEARS later have it come back -- usually when some event occurs like a bad pneumonia. That's because this form of malaria can 'hide' in the liver. If in doubt see an ID doc.

4 weeks once no risk: Regarding Doxycycline as a malaria "preventive", you need to complete 4 weeks of daily therapy once you've left the area of malaria risk (eg: no further risk of being bitten by a possibly infected mosquito). Depending on where you are, you likely need to continue for four more weeks as there is likely risk in your port. If not, then you only need to finish up the remaining two weeks.

Malaria pills: Check with your local health department they get updates from the cdc bi-weekly. Costa rica  has been an area where malaria prophylaxis has been recommended( my daughter has been to costa rica) you usually take a pill weekly starting 1 week before travel and 6 weeks after return. See your doctor or health department to get the medication. It is not a shot.

Mosquitos: No. Certain countries have certain strains of mosquitoes that carry certain kinds of mosquitoes. In the US, mosquitoes do not normally transmit malaria.
