Physical exam: Rather than look to tie symptoms together, you should get a physical exam from an orthopaedic surgeon. You may have spinal disease compressing nerves that supply the sensation to those areas I.e. Sciatic, obturator, sural nerves. Unlikely: It is more likely a neuropathy - ie a nerve being pinched in your low back.Common causes are a disc bulge or herniation, but even more often it is due to dysfunction of a vertebra which could easily be fixed by osteopathic manipulation.A good history & phys, incl hands-on osteopathic exam should help determine the cause &, manipulation could fix certain problems immediately.

See your doctor!: Back and leg pain could result from compression of the sciatic nerve, but it would be unusual for a uterine fibroid to cause this. You need to see your gynecologist for evaluation of your pelvic pain and urination; the back and leg pain could be related or a separate problem. Sounds like sciatica: You are describing pain along the sciatic nerve.  It may be present other times but your period masks it.  Also, in certain instances (pregnancy, enlarged pelvic veins, uterine fibroids, etc) the enlarged uterus can push on the sciatic nerve.

Sacroiliac joint: Late teen female with abdominal and pelvic pain associated with leg pain might have concern for IBS, sacroiliac joint disorder and sciatica.  These symptoms as well as PMD , also called PMS, can arise in women with Joint Hypermobility
Syndrome.  An Osteopath evaluation might be useful.
