UTI: Both are not primary choice. Antibiotic course depends on bacteria growing in urine, detecble via urine cukture. Bactrim (sulfamethoxazole and trimethoprim) or Macrobid ot Ciproflixacin are usually first choice. Please see doc. Thanks. Xf

Infected bladder: It is not normal to always be infected.  Big difference between a bladder infection and a kidney infection   kidney infections usually much worse with high fevers , back pain and feeling very sick.   Bladder infections common in women and usually are more frequent when women become sexually active.  Sometimes bladder "infection" is just irritation.  Avoid tight fitting panties or sports shorts.

Possible: Did you complete antibiotic course? Sometimes it takes time to get back to normal. Most of the time once you complete antibiotic, your symptoms should go away. If you continue drinking lot of water then you may feel reflex to urinate. Needs to consider diabetes as well if your symptoms persist after being treated for UTI long time ago.

Alkaline urine: If there is an obstruction between the infection and the bladder, the urine may be unremarkable.  A very alkaline urine with infection suggests proteus which produces urease that splits urea into ammonia that makes the urine alkaline.

While both can: Cause malodorous urine, kidney infections are more serious: pain in side or back, very high fever, chills, and sepsis.  Bladder symptoms are frequency, urgency, burning.  The latter, if neglected or treated wrongly, can lead to an upper trcat infection.

Yes: Bladder infections in pregnancy are more likely to progress to kidney infections than normal. Kidney infections may also cause preterm delivery, pneumonia or bacteria in the bloodstream. If you have a bladder infection and develop fever, pain going into your back or symptoms like the flu, you need to let your doctor know immediately.

Burning: Burning with urination is one of  the most common symptoms and bad urine odor.  Some people experience discomfort or pain in the low abdomen.   If the infection is in the kidneys sever back pain and fever will be present.  Both should be evaluated by a physician and treated.  Sometimes if the infection is in the kidneys hospitalization is required.
