65 year old male heart patient with previous heart attack 2 months ago with successful stenting of lad. taking bisoprolol, ticagrelor and apixaban to treat a small lvt. recently found gall stones in gallbladder with ultrasound. can i do ercp?

Accepted Answer

: It is not advisable to discontinue your anticoagulants because of the recent stent placement and,I assume a recent dvt. It's an easy decision to wait 4 more months before stopping ticagrelor if the gallstones aren't causing symptoms. Apixaban may be stopped sooner at your physicians discretion. However needing an ERCP suggests there are problems. Collaboration between GI&amp;cardiology required.

why is it that trapped gas feels like you are having a heart attack? when i pass i feel fine.

Can't be too sure...: No one symptom definitively indicates a heart attack is or is not in progress (many people make the fatal mistake of waiting out atypical heart pain). In fact, "heart pain" is often indistinguishable from esophageal, pericardial, mediastinal, gastric, and chest wall distress. By the same token, where in your gut you retain "trapped gas" may give you very different sensations. See doctor to be sure.

Yes : Would definitely recommend that you seek timely medical attention if you experienced the above mentioned symptoms. Heart attack is definitely a possibility and it is not the gas causing the symptoms.

Yes; keep healthy!: Yes, but not necessarily soon. It seems you had clogged arteries, and surgery bypassed some of the more clogged areas. There are other areas that are likely less clogged, though still clogged, that were not bypassed. These areas can present a problem in the future. You are doing the right things by keeping BP and cholesterol down; with your doctor's guidance, try to add some diet and exercise!

CABG: During a CABG procedure, the clogged portions of coronary arteries are bypassed with using short sections of a vein removed from one of your legs called the great saphenous. It is like building an overpass to redirect and flow of blood away from the clogged section and deliver it to the compromised heart muscle tissue.

The : The combination of chest pain, left arm numbness and increased symptoms when you are more active seems to suggest a possible cause of your symptoms being related to your heart.  I think it would be reasonable to pursue testing which may be sensitive enough to pick up a subtle problem - perhaps a congenital abnormality of a coronary artery or a predisposition to a marked tachycardia (rapid heart rhythm).  A stress echocardiogram or a holter monitor might be of value.  I would recommend discussing this in person with your heart specialist or another cardiologist.

It might: It may give you acid/indigestion that may feel exactly like a heart attack. But if you are at risk otherwise, better safe than sorry and get yourself checked.

Get eval: Doubtful it is heart at your age, but you need to get checked to be safe, as you should not have to deal with your pain for 3 days now.  Get an exam and you will be helped.  Best wishes.
