PVC: Heart very complicated electrical system. First why taking metoprolol. Is is succinate  which is once day or other which is every 8 hrs, do have heart condition or atrial fibrillation? Pvc are common but need know if there is underlying pathology. Consult ur dr. Or cardiologists for testing . They can be result of more serious problems. Beta blockers not first line for blood pressure. Future give.

Diltiazem: As a rule diltazem which is a calcium channel  blocker is not great for pvcs so i would try another beta blocker ie Coreg , atenolol or inderal (propranolol).

: Losartan is reasonable next one.  Every high blood pressure is different and their own experiment to find best combination of Diet &amp; meds. If you were not recommended to try the DASH Diet eating plan do it now after talking with your Dr. This lowers BP as well a/better than most single BP meds. Take BP at home &amp; ask for tests to find out why BP so difficult. Ask about aldosterone/renin ratio test.

PVC's: You could try magnesium for the PVC's or increasing your water intake and exercise to try and decrease the PVC's.  Any of the beta blockers can cause weight gain, so try to use lower doses.

on 12.5 mg metoprolol er for benign pvcs and cardio wants me to add 30 mg diltiziam 2x a day. im afraid to add this drug. is it safe? my pvc burden as of december was 1.7% and i can tell its much lower now in the metoprolol. need advice?

Yes but don't: At worst the pills may have lost some potency.  However, there is not good medical reason to use a beta blocker for PVC's.  The extra beats are harmless unless you have underlying weakness of the heart or previous heart attack.  In those cases, a person should take a beta blocker drug every day.  Otherwise, I'd urge you not to use the med which has no benefit in simple PVC's.

Blocks adrenalin: Metoprolol is a beta-blocker which blocks beta receptors in the heart and body.  It is useful in blocking the effect of Adrenalin on the heart making it less likely for irregular heart beats such as pvc's to occur.  Taking a long acting form of metoprolol (succinate) at night may allow for control of the pvc's all day and decreases fatigue a side effect that some people experience.

Why?: What are your fears? 25 mg is very low dose, many doctors start at 100 mg.  it would be a waste of time and a waste of your medication to decrease your dose to 1/4.  You need to discuss your specific concerns with your prescribing doctor.

Tartrate v Succinate: The blood pressure medication, metoprolol, comes in two forms, tartrate &amp; succinate, ie metoprolol tartrate (MT) &amp; metoprolol succinate (MS). MT is dosed twice daily - think T for twice, while MS is single daily dose - think S for single. Hope that helps! Oh and besides controlling blood pressure, metoprolol, as a member of beta blocker family, keeps your heart from racing away b/c PVCs.
