See below: You are very young age wise to get a smoker's cancer  ordianrily it takes at least 20 years.  So the thing in your thrat may be an irritation from something you ate or in fact the smoking may cause you to feel this way but it is not cancerc.

Direct vision: The best way to diagnose this problem is direct visualization with a nasopharyngoscope  and take a biopsy of the suspected lesion. The pharynx (throat) is accessible and imaging tests such as a CT orMRI would be useful to determine extension to adjacent structures and if lymph nodes are enlarged. Nasopharyngoscopy is generally performed by an ENT specialist.

If these are: discrete nodules, next step is fine needle aspiration.  Depending on the size of the thyroid gland and other studies, you might need to undergo thyroidectomy.  Consult with your endocrinologist for specific advice.  Chances of these being cancer is probably low.

Vague question: "throat cancer" is a vague layman's term which can encompass many different problems.  In general, most head and neck "throat cancers" are squamous cell cancers, which are commonly associated with smoking, drinking and hpv infection.  They can occur anywhere there is squamous mucosa: tongue, tonsil, palate, throat, larynx, etc.  See you ENT for an exam or screening.

Yes and yes: Both throat and thyroid cancer can spread to the central neck lymph nodes, which are essentially next to the thyroid bed.  Smoking and alcohol use will increase your chance of having throat cancer.  Given your history of thyroid cancer, this is probably more likely.  A thyroid scan or fine needle biopsy of the area can help answer the question, along with checking a serum thyroglobulin.
