Your sister MUST see her primary doctor as soon as possible.
The primary doctor can begin to improve her diabetes but she must also see the specialist who will work on the upper arm pain.

I don't think: One can assume that all of these complaints are as a result of diabetes.  Each issue you list needs to be taken into consideration along with your other medical history, activity level and any other medical issues you have along with an exam.  I would recommend an evaluation by your treating doctor.

Upper arm pain: Consider referred cervical pain, referred heart problems, sprains, strains, rotator cuff tendonitis or tearing. Most ominous would be the one related to the heart. Check with er or your pcp asap for this.

Many things: Problems like these can only be correctly handled by your doctor in person.  He/she needs to listen to you, perform an examination and possibly run labs or other tests. That's the only way he/she can find out what's going on and what to do about it.

Probably the muscles: And/or ligaments are pulled/strained. Occurs if one does not do warm ups or increases the weights too rapidly.
Take some pain meds (nsaids), do stretching exercises, and slowly build up the weights you lift.
If no improvement, then see an orthopod and get a formal pt and I am sure you'll get better.

Arm pain: Many causes. Consider tendonitis, tendon tears, overuse, referred cervical spine pain, carpal tunnel, cubital tunnel, radial tunnel, bone injury, contusion, infection, fibromyalgia, inflammation, tumor, etc. Check with orthopedic surgeon.
