Refusing formula : Formula is usually meant for babies under 12 months old, if your child doesn't like the taste,  you could try to give him regular milk. The biggest difference between the two is that toddler formula has extra vitamins, minerals, and nutrients. But most children don't need this supplementation and are fine with solid foods and regular milk.

Dehydration: you can tell if his urine is very dark or have strong smell that your son is not adequately hydrated, unless he gets his fluids in other forms (juices, sodas, etc). in either case that is not good and not healthy on the long term (kidney problems, mineral deficiencies). try to talk to him on why it is importance and create a log. minimum intake should be around 1.5 liter per day.

Please don't: Theoretically, yes, if the nesquick/milk combination is not too thick.  However, please don't feed your one-year old nesquick, or any other sugary food.  Nesquick and other similar products have very little (if any) nutritional value, and just put the child at future risk of dental cavities and poor nutrition habits.  When the child is older, sweet foods can be had occasionally as treats.

Breastfeeding: If you were breastfeeding your baby was not constipated as you think. Rather, breast fed babies can go several days without having a stool. Yes, one cup of coffee a day is fine.

Fiber and water: Increasing fiber intake combined with adequate free water is the basis of constipation prevention, furthered by avoiding binding foods including dairy, bananas, starches, etc.  Lactulose is available by Rx for children.

Milk: It is recommended to switch to whole milk at one year.  The recommended amount is 2 cups a day (to provide adequate vitamin d) but not much more.  When kids are "milkaholics" they actually can become iron deficient.  I'd shoot for 2 cups (16oz) per day and stop there.  (in a cup - get rid of the bottles too.).

Milk: Toenail breaks in an infant usually normal. A one year old does not need to drink milk if he is eating other dairy products.

Yes, of course: Cow's milk is for cows. It is a mistake to think that this is the only source of calcium. He should be gradually introduced to table food, concentrating on fruit and vegetgables using junior food and graduating to family food.

: She is not digesting the milk well. An infection is one cause. Switch to almond milk and provide an infant probiotic daily. Watch for signs of fever and diarrhea. An allergic reaction is another possibility, and a blood test may be helpful to determine whether she is allergic. Watch for signs of rash .

Focusing : Focusing on your heart is not a bad idea. You could be dealing with a heart problem, anxiety, or both. There are  heart conditions (namely abnormal heart rhythm, or valvular problem) which could cause your tachycardia and chest pain. You may benefit from an evaluation with a 24 hour holter monitor (device you wear for 24 hours that records abnormal heart rhythms) and/or echocardiogram (ultrasound picture of your heart). This can be arranged by your primary care provider (pcp), by referring you to a cardiologist. This specialist will decide if these tests or any additional tests would be helpful.

At the same time your anxiety needs to be addressed. Are you seeing your pcp for this or a psychiatrist? There are medications that can help alleviate or relieve your symptoms.

Lastly your stomach issue needs to be addressed as well. You could be suffering from gastritis(inflammation of the lining of your stomach), your pcp will likely work up this problem and start treatment.
