Malignancy would be a rare cause.

Skin flora: It's the same as anywhere else on the skin.  There are a number of bacteria.  The most common are staph epidermidis and other staph species.  Strep mitis is frequent and strep pyogenes is less common.  Priopionobacterium is common as is acintobacter. There are others that are not as frequent. Fungal growth from candida is also very common and there are other fungi to a lesser degree.

Might be hives: Unless there is a direct exposure of something to his ears that is causing them to react, this is likely hives due to allergy, virus, or a sensitivity.

Dry patch on face: Several things come to mind.  He might need more omega 3 fatty acids in his diet or supplements.  It might be a patch of fungal infection.  It could also be representative of a food sensitivity.  It wouldn't hurt to stop gluten and dairy for a month and see if that helps.
