Many things: Generally toenail deformity are indicative of very old and sometimes minor injury or damage to nail bed. .  They can also be from disease such as athletes foot or circulatory problem.  Usually you won't know what caused them.

Get it checked: In order to treat a lesion, you must first know what it is. It could be a corn, a wart or some form of dermatitis or other skin anomaly. See a podiatrist who can diagnose it properly and recommend the correct course of treatment. I do not recommend self-treatment with otc products. Many contain salicylic acid and can cause painful chemical burns.

R/O PLANTAR WART: Plantar warts are caused by HPV AND CAN OCCUR ON HAND AND FEET. They can also involve the nails. Get eval by Derm. Can be treated with topical Aldara/laser/cryotherapy.

Without seeing hard: to comment. Sometimes, the remaining portion of the toenail needs to be removed, othertimes it can be left....decision is based on pain, blood under nail, infection etc...

Hard to say: It is hard to say without directly seeing it.  The nail grows in part from the nail bed and so if the nail bed was injured, it may produce an abnormal shape to the nail.  Consider being seen and thoroughly evaluated.

Sounds like the: problem might be hives (an allergic reaction.) Or possibly a fungus infection. You need to see a health care professional for an examination and some testing to figure this out.

Most likely benign: This lesion most likely is a viral wart, or other benign  lesion, but should be removed +/- a microscopic tissue diagnosis.
