Possible: Insect bites are possible but need to make sure it is not ringworm. Your description fits ringworm well. It is a highly contagious fungal infection of the skin and scalp spread by contact. Can be itchy and get crusty. It is treated with topical anti fungals like Lotrimin, Miconazole, or several others. In severe forms or locations not suitable for topicals you may need oral antifungals. Send pic

Lyme disease: This is caused by deer-born ticks and can have serious long term consequences if untreated. If you live in an area that has deer or there has been reports of Lyme disease, I would certainly recommend testing. Even if it is negative in a 3 yr old, but there is a medium suspicion for Lyme, I would argue that it is worth treating with abx (doxy, amox). The risk of tx far outweighs complications of dz

Nerve: Check for nerve compression including the wrist and elbow areas.  Consult an orthopedist and get some nerve conduction study done.  This is not my specialty and hope others in HT will comment on this as well.

Not from stress: I can tell you they are not from stress, but i can't tell you if they are from bed bugs or not. Perhaps you should collect some of the critters and take them to your family doc or a dermatologist for identification and further elucidation as to the cause of the rash. Depends on what you: Are referring to. Chiggers can. Chigger bites often have a small red in the center of a welt  ; occur in areas where skin is tender like the  crotch, armpits, beltline, backs of knees, ankles. They are extremely itchy.
