Thumb bone spurs: Any joint can get bone spurs. Bone spurs or osteophytes are a common feature of arthritic change in a joint. The common findings of arthritis on an X-ray are  are joint space narrowing, sclerosis ( or whiter bone on X-ray), bone spurs, bone cysts . Clinically or in real life this may be seen as clicking, swelling, pain , tightness, beginning with end range of motion symptoms.

i fell and fractured my thumb and some bones in my palm when i was 21. now at the 32,  i have constant pain in my thumb, can't straighten it completely and when i try to there is a tight stretching feeling, and have developed a knot over the thumb joint.

Thumb pain: sounds like traumatic arthritis.  Physical Therapy, therapeutic modalities, and medication may help you.  Injections can also help for the knots.

had a bone spur removed & joint fused in my hand a year ago and it still isn't pain free. think it was a cmc joint. is this normal to have pain still?

Depends: Mild achy pain is not uncommon, however, more severe pain is not normal.  You should make a follow up with your orthopedic hand surgeon for a reevaluation and possibly need new imaging studies.

i have bone spurs on xrays. going for mri. dr. thinks i may have torn my cuff. would this make back of my shoulder numb? also would my thumb be numb?

No: This an indication of having neck problem disc (c5- c6) level and may be than one.
Ask doctor to run x-ray of the neck area.

why can't my thumb joint be surgically cleaned out?

Technically it can: But it is usually of little benefit, because of the small size of the thumb joints.  Depends on which joint as well.  If you have pain or DJD in your basal thumb joint or cmc joint it can be cleaned out with a procedure called a trapezium resection and suspensionplasty.  Great surgery 97-99% success rate.  The other thumb joints can either be fused, but this sig. Decreases thumb function.

RICE: Rest, immobilization, cold and exersice. Thumb joint pain, especially at the base, is usually arthritis. I start with a brace and oral steroids and if pain persists, injections and therapy follow. If i can't relieve the pain and x-rays show arthritis, i will procede with arthroplasty. Surgery consists of removing the arthritic bone and replacing it with tendon.
